Louisiana law requires immunization against measles, mumps, rubella, and tetanus-diphtheria. LSU also requires a completed tuberculosis questionnaire for all first-time LSU students.
Louisiana Residents: LSU automatically receives immunization information for in-state students. In addition to having up-to-date immunization records, you must also submit the TB questionnaire to complete the requirements. You may complete this form, as well as check your immunization compliance status via the Patient Portal.
Non-Louisiana Residents: You must submit the Proof of Immunization Compliance form, which must be completed by a physician or healthcare provider. No separate documents or attachments are accepted. You may upload the completed form via the Patient Portal.
Use your myLSU credentials to log in to the Patient Portal.
In order to complete registration, you’ll need to submit final official transcripts from all colleges and universities attended.
You must contact the registrar's office of your college or university and request they send your final transcript directly to LSU. This is the only acceptable form of submission. If they ask for an email address to send to, please use admissions@lsu.edu.
To see which college transcripts are missing, view the checklist in your LSU Admissions Portal.
